Aflac is the company that administers our Life & AD&D benefits.

You can elect up to 5 times your annual earnings in Team Member Voluntary Life Insurance, up to a maximum of $750,000. When you enroll for at least 1 times annual earnings in Voluntary Life Insurance for yourself, WFM automatically provides you with additional Team Member Basic Life and Basic AD&D Insurance in the amount of 1 times your annual earnings—at no cost to you.

If you enroll for Team Member Voluntary Life Insurance when you first become eligible for coverage, you do not have to submit proof of good health—called evidence of insurability (EOI)—if you elect coverage of $500,000 or less. You may be asked to submit (EOI) during Annual Enrollment if you elect coverage for the first time, increase your coverage, or elect coverage above $500,000.

If you enroll and are approved for Voluntary Life Insurance, the company automatically provides you with AD&D coverage at no additional cost. Your AD&D coverage is equal to your Team Member Basic Life Insurance (1 times pay), plus any Team Member Voluntary Life Insurance you elect for yourself.
